Who Can Refer to Resolve?
We welcome referrals from:
• General Practitioners (GPs)
• Hospitals and hospital discharge teams
• Allied health professionals
• Support coordinators and support plan managers
• Case managers and care coordinators
• Families, carers and individuals seeking support
• Self-referrals
We work collaboratively with participants, families and referring professionals to understand individual needs and determine how Resolve Community Care may be able to assist.
How to Refer
Referring someone to Resolve Community Care is simple. We welcome referrals from healthcare professionals, support coordinators, case managers, families and individuals seeking support.
To make a referral:
• Complete our referral form using the downloadable form below
• Email the completed form to resolvecommunitycare@gmail.com
• Prefer to discuss the referral first? Contact us directly to discuss the individual's needs and how we may be able to assist
We will review each referral and contact the relevant person to discuss service availability and next steps.
