Resolve Community Care provides person-centred community nursing and clinical support tailored to the individual needs of people living with disability.
Our registered nurses and support workers focus on safe, evidence-based care that promotes health, comfort, dignity and independence. We work collaboratively with participants, families and other healthcare professionals to support individual care needs.
Our nursing services may include:
• Community nursing and clinical support
• Continence assessments and management
• Medication management
• Bowel care
• Catheter care and ongoing clinical monitoring
• Wound care and other nursing interventions, where appropriate
• Individualised nursing care planning and clinical documentation
